Diabetes Coalition

The Diabetes Coalition of Northeast Ohio (DCNEO) is a community-based collaboration of physicians, pharmacists, diabetes nurse educators, public health officials, Ohio KePRO representatives, employers and other stakeholders throughout Northeast Ohio whose goal is "to improve the quality of diabetes care delivered in our community through partnerships and collaboration and by increasing awareness, emphasizing prevention, and implementing consistent, evidence-based care management in a cost-effective manner."

The DCNEO engages providers, payors and patients through the following activities:

• Diabetes Care Resource Guides
• Diabetes Care Flow Sheets for Physicians
• Health Fair Booth
• Patient Education Meetings
• Support of programs such as Bridges to Excellence

The Diabetes Coalition serves all of Northeast Ohio and typically meets the 3rd Wednesday of each month from 8:00 - 10:00 am. Meeting locations alternate between the Stark County and Summit County Health Departments. There is no cost to be associated with the organization and new members are always welcome.
